Eligibility Request Form
Click here to complete the Eligibility Request Form. This form is required for any student requesting an excused absence related to a 4-H activity. In accordance with UIL guidelines, students must maintain academic eligibility (“no pass, no play”). Once the form is submitted, our office staff will ensure it is delivered to the appropriate school.
