Overview

Submit insect samples for identification to the experts in urban entomology.

Carefully collect insects (live or dead) and place them in in vial or other leak-proof container. Please make every effort to collect multiple specimens, if possible. If collected alive, place insects in a freezer until dead. After this time period has elapsed, remove any debris from the container and add enough isopropyl alcohol to submerge the specimens. Place the container inside a sealable bag. Place the specimens and this completed form in a box and ship to the address below.

NOTE: Prior to shipping specimens, you must declare that the contents contain alcohol.
